How to Understand the 24-Week Efficacy Numbers

Participants in the trials started with a mean baseline SALT score of 83.9. This score indicates extensive missing hair prior to the intervention. Women represented 59.0% of the combined study population. During the trial, participants received once-daily upadacitinib at either 15 milligrams or 30 milligrams, or a placebo.

The primary goal was reaching a SALT score of 20 or lower by week 24. This target means no more than 20% of scalp hair is missing, representing roughly 80% coverage. In the UP-AA1 trial, 45.2% of patients taking 15 milligrams and 55.0% taking 30 milligrams achieved this coverage threshold. In the UP-AA2 trial, those respective outcomes were 44.6% and 54.3%.

These numbers demonstrate a clear statistical separation from the baseline. For comparison, the placebo groups saw only 1.5% and 3.4% of participants reaching the same coverage threshold. The researchers reported that this primary endpoint began to separate from placebo as early as week eight. The higher dose produced the target coverage in slightly more than half of participants.

However, complete scalp regrowth was notably less common than substantial coverage. Complete restoration, defined as a SALT score of zero, occurred in 20.3% of the 30-milligram group in UP-AA1. The UP-AA2 trial showed 22.5% of the 30-milligram group reaching complete regrowth. The 15-milligram dose produced complete scalp regrowth in 14.1% and 13.1% of participants across the two trials.

Why Longer-Term Data and Medical Context Are Essential

While the clinical response is notable, these findings carry important medical caveats. The 24-week observation window is relatively short for an ongoing autoimmune condition. The study authors specifically noted that longer-term efficacy and safety data are still needed. Furthermore, the trial excluded adults older than 64 years and people whose current episode lasted more than eight years.

Safety monitoring is a crucial part of taking any immune-modulating medication. During the 24-week period, serious adverse events were reported in 1.6% of the 15-milligram group and 2.3% of the 30-milligram group. This compared with 0.4% in the placebo group. The most frequently reported events included upper respiratory tract infections, acne, and elevated blood creatine phosphokinase.

General treatment-emergent adverse events were fairly common across all groups in the study. These events occurred in 57.1% of placebo recipients, 62.7% of patients receiving 15 milligrams, and 67.1% of patients receiving 30 milligrams through week 24. While side effects were documented, the researchers noted that no deaths were reported during the 24-week treatment period. The safety profile was considered consistent with the established approved uses of upadacitinib.

Acne was reported in 11.7% of participants receiving 15 milligrams and 16.3% receiving 30 milligrams. The placebo group experienced a much lower acne rate of 3.6%. Upper respiratory tract infections occurred in 9.5% of the 15-milligram group and 13.2% of the 30-milligram group. These figures highlight the necessity of regular medical supervision.

The regulatory context also requires careful attention from patients. A September 2026 dermatology review reported that upadacitinib was not FDA-approved for alopecia areata at that time. The JAMA Dermatology paper stated that baricitinib, ritlecitinib, and deuruxolitinib were the currently approved JAK-inhibitor treatments. The paper identified ritlecitinib as the only one of those three approved for adolescents.

A separate dermatology review noted that off-label use of upadacitinib is sometimes considered when patients have another condition for which it is indicated. Finally, AbbVie funded the research and provided writing support for the study.
