Participants received either 5 grams of creatine monohydrate daily or a placebo for 12 weeks. The trial found no significant creatine-related changes in measured hair outcomes.

While controlled trials are helpful, real-world outcomes often differ from clinical settings. When evaluating benefits for postmenopausal women, a 2026 meta-analysis analyzed seven randomized placebo-controlled trials involving 608 participants. Researchers found an average 0.37-kilogram advantage in lean mass and a 7.5-kilogram advantage in leg-press strength for those taking creatine. The clearest benefits occurred alongside resistance training at doses of at least 5 grams per day.

The review found no significant increase in total adverse events, gastrointestinal events, weight gain, or renal or hepatic complications compared with controls. In that two-year trial, creatine did not improve femoral-neck, total-hip, or lumbar-spine bone mineral density. The trial reported some secondary improvements in walking time and femoral-neck geometry, but these do not establish creatine as an osteoporosis treatment.
